Hand Hygiene

This report recognizes the 5 Moments for Hand Hygiene, which are: before touching a patient, before a procedure, after a procedure or exposure to body fluids, after touching a patient, and after touching a patient’s surroundings.

When you make a hand hygiene observation, note which of these moments you observed and check their boxes in the form below, which then allows you to note whether hand hygiene was performed or not for that moment. After submission, you can click the button to submit another observation. Only submit one employee observation at a time.

Example: As you are walking by a patient room, you see a nurse walk inside and touch the patient without washing their hands first. Then you see them wash their hands before leaving the room. You would check the boxes for “Before touching a patient” and “After touching a patient” on the “Select the hand hygiene moments you observed.” question, then you would mark “No” on the question “Was hand hygiene performed before touching a patient?” and “Yes” on the question “Was hand hygiene performed after touching a patient?”
